Assertive Community Treatment Supervisor

General Statement of Duties:

To ensure the ongoing operation of Assertive Community Treatment (ACT) services, supporting individuals with serious and persistent mental illness in a recovery-oriented environment. The position provides clinical and administrative supervision for professional staff positions and ensures that services are in conformity with State Licensure and Medicaid regulations.  The incumbent in this position is responsible for the coordination, implementation, and maintenance of ACT services. This position oversees all operations for ACT, including coordination of coverage of direct services. As part of the operational duties of ACT, this position may require occasional evening/weekend or holiday duties as needed.

The position is supervised by the Director of Community Mental Health Services. Regularly scheduled supervision is both provided and received, as well as ongoing consultation and assistance necessary to accomplish the goals of ACT. Incumbents in this position should have demonstrated capabilities and initiative in serving individual clients and program design and implementation.  Job duties require the ability to act independently, make decisions, interpret policies and procedures, consult and collaborate with other staff members, maintain client and personnel confidentiality, and provide leadership in the delivery of services to clients of the Board.

Major Duties and Responsibilities:

  • Ensures ACT services follow all regulations as identified by DBHDS and DMAS. Establishes and maintains fidelity to the ACT model as identified in the Tool for the Measurement of Assertive Community Treatment (TMACT);
  • Provides consultation and oversight to the development of new programs utilizing the ACT model at HRCSB;
  • Represents HRCSB in all ACT related meetings with other agencies or state entities;
  • Provides direct supervision to ACT staff, including the provision of clinical and procedural guidance.  This includes the establishment and monitoring of performance objectives and performance appraisals;
  • In coordination with other agency staff, develops and maintains protocols or admission policies regarding the admission and referral of individuals requesting services from the agency. Will participate as a team member in screening and triaging services requests;
  • Monitors the provision of services, utilization, revenue and disbursements, and adherence to agency policy in consultation with Director;
  • Provides direct clinical services to clients with severe and persistent mental illness;
  • Oversees staff scheduling to ensure that sufficient resources are in place to meet program or department demands, including ACT Team and coordination with Crisis Services. This would include monitoring attendance and punctuality of program staff;
  • Conducts daily team meetings in accordance with regulations to include the effective communication of protocols and changes, agency expectations, staffing issues;
  • Participates in the recruitment and selection process of staff directly supervised by this position, including screening and interviewing of applicants and making hiring recommendations in consultation with Director;
  • Monitors clinical documentation to ensure quality documentation standards and to assign clinical follow-up as appropriate;
  • Implements continuity of care procedures and coordinates the provision of ongoing services in observance with applicable agency initiatives;
  • Participates in statewide ACT and CSB training conferences to maintain the most updated knowledge related to the ACT functions and agency/community services;
  • Assists in data collection and program research projects as necessary;
  • Maintains relationships with community partners including law enforcement and hospitals to ensure continuity of care for clients;
  • Collaborates closely with other agency staff for the purpose of services facilitation and developing efficient procedures for getting new clients into services;
  • Participates in regular meetings with agency personnel as indicated to coordinate cases, services, and outcomes as a means of enhancing the provision of services;
  • Produces client related service records and maintains accurate and up-to-date documentation consistent with agency policies and procedures;
  • Performs other duties as assigned by supervisor which are consistent with the position and in compliance with agency policies and procedures.
  • Conducts meetings for the purpose of coordinating services to individuals discharged from state psychiatric facilities to this community;
  • Establishes and maintains relationships with other community agencies, advocacy groups and service providers;
  • Maintains and assures that staff of the program maintain effective working relationships with coworkers and customers based on courtesy, compassion and respect; insures that staff protect carefully the confidentiality of clients;
  • Demonstrates a desire and willingness to continue growing and learning professionally, as evidenced by participation in appropriate workshops, seminars, and conferences;
  • Makes effective use of available technology, including computers, e-mail, and voice mail in order to enhance customer service;
  • Performs other related duties as assigned that are consistent with the mission of the Harrisonburg-Rockingham Community Services Board;

Required Knowledge, Skills and Abilities:

Considerable knowledge of adult and geriatric psychology in mental health and substance use disorder areas; of primary models for treatment of individuals having these problems; of appropriate methodologies for assessing psychopathology; and, of crisis intervention theory and techniques, and their application to individuals in crisis.  Working knowledge of community systems and resources; and, inter-agency collaborating.  Knowledge of or experience with ACT model of treatment. Supervision experience preferred.

Minimum Education and Experience:

Graduation from an accredited college or university with a masters degree in human services plus 3 years of experience in a mental health,  developmental disorders or substance abuse setting or a related human services field; or any equivalent combination of education and experience which provides the required knowledge, skills and abilities. Professional licensure or Licensed Eligible required.

ACT Nurse

 

General Statement of Duties:

Assertive Community Treatment (ACT) Nurse (Registered Nurse or Licensed Professional Nurse) will provide in-home services to clients with severe and persistent mental illness. Position is responsible for meeting the nursing service needs of agency consumers by providing competent standards of care in direct and indirect care roles. The Nurse will assist clients with medical aspects of their care through administering and delivering medications, assessing for medication side effects, educating about medications taken to ensure patient’s informed consent, and providing education about symptoms management. The Nurse will work in conjunction with others on the ACT Team to provide comprehensive services for clients in their homes and community settings. This position involves a rotation of day, evening and weekend shift coverage including overnight and weekend rotation of on-call coverage. This position includes assessing service needs, developing treatment plans and providing assistance with access to community resources. Incumbent assists in obtaining medication and monitors response to prescribed medications and ensures that the medical staff has accurate information about the consumer’s response and/or difficulties related to the medication. The incumbent is expected to exercise a high level of confidentiality regarding clients and services. Receives regular direct supervision from ACT Supervisor, however, direct oversight of medical duties is performed by ACT Psychiatric Care Provider. Assures compliance with agency policy, client rights, confidentiality, licensure regulations and Medicaid regulations as applicable.

Major Essential Functions:

  • Assist in obtaining, safe storage, handling, and administration of IM and oral medications in compliance with state and federal law, department guidelines, and Community Service Board policy and procedure, as well as safe storage and handling of necessary medical supplies.
  • Prepares medication for regular delivery to client, checking medication orders for accuracy. As well as filling out proper documentation as per written protocol.
  • Practices good venipuncture techniques and administers intramuscular injections under the direction of the medical prescriber.
  • Provides basic medical assessment to consumers, including monitoring vital signs.
  • Monitors clients for development of side effects of medications, reporting this to the prescriber and documenting as such in the client case record.
  • Assists with calling in medications ordered by the prescriber to pharmacies as authorized by psychiatric care provider.
  • Assists psychiatric care provider with managing indigent programs and use of sample medications, as needed for clients.
  • Assists with arranging and coordinating laboratory testing as ordered by the psychiatric care provider.
  • Assists with obtaining prior authorizations from insurance companies for client medications as ordered by the psychiatric care provider.
  • Assists with therapeutic interventions in both individual and group settings to persons who are in crisis due to their mental health status.
  • Assists with admission process for individuals being admitted to the program.
  • Responsible for the cleaning and upkeep of the medication room and other program areas on a routine basis.
  • Provides medication, health, and wellness education on an individual and group basis to clients.
  • Provides training and support with activities of daily living, performs crisis intervention, and facilitates prescreening/hospitalizations when necessary. Assists and participates in all hospital diversion, hospital care coordination, and hospital discharge/re-integration services.
  • Documents client contacts and maintains client records in accordance with established board policy and procedure.
  • Assists with discharge planning and process, which includes ensuring clients are discharged with adequate supply of medications and the resources to obtain continued needed supplies.
  • Makes effective use of technology available to use.
  • Maintains effective relationships with co-workers and clients based on courtesy, compassion, and respect.
  • Participates in team meetings, clinical staffing and supervision.
  • Participates in professional development training, seminars, and conferences.
  • Is responsible for the duties as assigned that are consistent with established Board policy.

Required Knowledge, Skills and Abilities:

  • Thorough knowledge of the principles and practices of nursing;
  • Adherence to the code of ethics of the nursing profession;
  • Knowledge of principles, practices, and techniques involved in the diagnosis and treatment of individuals with mental health, and co-occurring disorders both individually and in a group setting.
  • Knowledge of mental health, substance abuse and co-occurring, treatment interventions both individually and in a group setting.
  • Ability to problem-solve and respond quickly to crisis situations. Ability to function calmly in stressful situations.
  • Basic knowledge about mental illness and the use of psychotropic medication in its treatment.

Physical Requirements:

  • May be exposed to allergenic environments in client’s homes, including extreme heat, humidity, dust, smoke, pet contamination, cleaning supplies, etc.
  • May be exposed to clients with infectious diseases.
  • Must be able to drive for long periods of time to visit clients in and out of the catchment area as needed.
  • Must be able to walk distances, climb and descend stairs, maneuver in and out of agency vehicles.
  • Prolonged periods sitting at a desk and working on a computer.
  • Must be able to lift/push/pull up to 25 pounds at times.

Minimum Education and Experience:

Graduation from an accredited college or nursing school with a degree or diploma in practical nursing, current licensure as a Registered Nurse (RN) or Licensed Professional Nurse (LPN) in the Commonwealth of Virginia, plus three years of nursing experience which includes experience working with mental health and/or substance use disorder clients. Must be eligible to register as a Qualified Mental Health Professional (QMHP) or QMHP-trainee with the State of Virginia. Staff member is responsible for submitting proof of license renewal to the Human Resources office. Possession of a valid driver’s license and acceptable driving record as reported by the Department of Motor Vehicles are required for persons using a personal or agency vehicle in the performance of their duties.

Work Schedule: ACT is a 24-hour program and flexibility is required in order to meet and respond to coverage needs and individual residents’ needs. Shifts will vary to meet the needs of the program. Full-time employees are required to work 40 Hours each week. Monday through Friday- day and/or evening shifts. Weekends and Holidays- evening shifts on a rotating basis with ACT team members. Overnight and weekend on-call on a rotating basis with ACT team members. Regular and reliable attendance is required.

Data and Business Analyst

Distinguishing Features of Work:

A primary goal is to ensure the timely and accurate collection and processing of data related to CSB clinical, administration, and service delivery, and report accordingly to state regulators. This position submits reports to DBHDS per the annual Performance Contract and any other required state reporting. This position works with a wide range of CSB staff to achieve desired data quality to assist the CSB in complying with state reporting requirements. The incumbent is familiar with the agency Electronic Health Record (EHR) system, and secures necessary data from that system. The incumbent will compile and distribute the monthly Agency Performance Report (APR) and other internal reports for management use.

The incumbent will evaluate available data and provide reports to CSB management for business use, including tracking State dashboards. The incumbent will assist in the analysis and utilization of data and outcomes related to services. The incumbent will require independent judgment, the exercise of a high level of discretion, and the use and application of data review, analysis and reporting techniques, such as BI. The incumbent will possess the ability to comprehend and apply regulatory and policy requirements necessary to meet data and reporting requirements. The incumbent must possess excellent verbal and written skills and be able to organize and prioritize time sensitive tasks. Incumbent will interact with directors, supervisors, clinical staff, IT and administrative staff as needed to respond to questions, provide feedback, and to ensure high quality data reporting.

 

Major Essential Functions:

  • Performs business-critical processes to achieve the required service and data integrity; includes related data entry and data processing responsibilities.
  • Provides guidance on data collection processes, standards, and automated system reports to improve data quality, provides technical assistance and/or re-training.
  • Identifies missing data and follows up on its collection; monitors the integrity and reliability of data collection processes; works collaboratively with other stakeholders to enhance data collection processes for accuracy and thoroughness within staff time/resource constraints.
  • Works collaboratively with IT staff in researching and resolving data and/or report errors/problems; as needed, performs gross data output checks of State Performance Reports, CCS reports, and other state extract reports. Data analysis can include staff time allocation as requested.
  • In coordination with the Manager, Director of Administrative Services and Executive Director, establishes and monitors performance against Performance Contract targets; generates and submits these and other assigned reports.
  • Contributes to the state submission of CSB accountability reports as required under the Performance Contract and/or General Assembly action.
  • Maintains a thorough understanding of state data reporting and outcome measures, providing analysis and follow up as applicable to QI responsibilities and Performance Contract or General Assembly mandated services.
  • Becomes proficient in use of agency Electronic Health Record (EHR) system, exporting required data for required reports.
  • Performs or coordinates the processing of data in support of monthly Agency Performance Report, works with affected staff to maximize the accuracy and utility of these reports.

Required Knowledge, Skills and Abilities:

  • Knowledge and experience in data analysis and reporting required, including the use of application such as Power BI. The ability to meet date-certain deadlines and manage routines with minimal supervision; liaison abilities, including flexibility and knowledge of various administrative systems; ability to engage in appropriate independent actions and take appropriate initiative per agency policies, federal/state personnel law, and related regulations ability to balance competing principles in data management; sound judgment.
  • Familiarity with healthcare reporting and regulations, especially as related to outcome measurement principles and techniques.
  • Technical ability with data reporting background, with either an Electronic Health Record (EHR) system, BI, or related experience.

Graduation from an accredited college or university with a degree in public administration, computer science or a related area; Possess strong knowledge of databases and business intelligence tools. Supervisor of Community Crisis Services

Major Essential Functions:
  • Provides clinical and administrative supervision to the community crisis teams.
  • Meet regularly with ES and Adult and Children’s Community Crisis staff for supervision and consultation
  • Orient and train new staff
  • Address any personnel issues in consultation with Senior Manager and Director as appropriate
  • Will dispatch Crisis Response Team or respond as necessary

  • Monitors and provides consultation with the MOT program.

  • In consultation with supervisor, provides back up to After Hours Emergency Services staff.
  • Provides and is available for consult to after services staff.
  • As needed, will help provide coverage for after hour shifts.

Infant Toddler Developmental Interventionist

Major Duties and Responsibilities:

  • Assesses children in a variety of developmental domains (as determined by the Local System Manager), using the tools and test procedures approved by the Local System Manager.
  • Participates in multidisciplinary assessments and team meetings for individual children.
  • Provides developmental services to assigned Infant & Toddler clients.
  • Consults with a wide array of other professionals to provide and receive information regarding medical, developmental, and/or human services issues.
  • Participates in staff development activities required by the Local System Manager.
  • Participates in staff meetings and team development.
  • Adheres to regulations and standards established by or for compliance with human rights regulations, agency licensure requirements, confidentiality statutes, third party payer rules, professional code of ethics, etc.

Good interpersonal skills with the ability to teach and to communicate clearly and effectively, both verbally and in writing. Ability to work and relate effectively to persons from a wide variety of educational and socioeconomic backgrounds who may vary markedly in their level of sophistication regarding infant and toddler development. Exhibits consistently good judgment and a high level of independence and initiative. Ability to work effectively as a team member and with a variety of community agencies. Basic computer literacy and word processing skills preferred.

ACT Clinician

Major Essential Functions:

  • Assists in the development, implementation, and maintenance of ACT team services.
  • Collaborates with other members of ACT Team to provide comprehensive mental health and supportive services to clients.
  • Participates in daily ACT treatment team meetings to discuss client treatment goal and needs.
  • Provides treatment planning, direct counseling, individual and group therapy, and case management to consumers with mental health and/or substance use disorders, individuals with multiple challenges that interfere with functioning; and their family members or identified supports.
  • Provides goal directed interventions that provide skill development to individuals who have serious and persistent mental illness so that they can reside independently in their communities in the least restrictive environment. Interventions may include: helping consumers acquire basic skills in symptom management and adherence to psychiatric treatment plans; assisting consumers in developing medication adherence skills; helping consumers acquire basic functional skills and appropriate behavior related to the individual’s health and safety; helping consumers with individualized training in development and appropriate use of social skills and personal support system; and assisting consumers with skill development for improved personal hygiene, food preparation, and money management.
  • Performs assessments, determines client service needs, and develops treatment/service in coordination with ACT team.
  • Provides client-centered treatment utilizing various modalities including basic principles of motivational interviewing, harm reduction, and integrated dual-disorder treatment to encourage harm reduction and sustained recovery. Support may include linking clients to appropriate substance use recovery services in the community as appropriate (i.e. AA, NA) and other natural supports.
  • Provides support to clients and families during times of crisis, and links clients to appropriate crisis stabilization services.
  • Coordinates assessment of clients in need of crisis stabilization, inpatient residential/detox treatment in cooperation with ACT team supervisor.
  • Assists in coordination of admission and discharge from inpatient treatment.
  • Collaborates and liaisons with law enforcement officers, local hospitals, medical providers, and others in the planning, provision, and evaluation of these services.
  • Produces client related service records and maintains accurate and up-to-date documentation consistent with agency policies and procedures. Maintains detailed documentation that is commensurate with requirements for Medicaid reimbursement and licensure regulations.
  • Receives regular individual guidance and supervision from ACT Supervisor.
  • Collaborates fully with psychiatric care provider to dispense, administer and monitor client medications. Delivers medications to clients in their natural environment, as appropriate.
  • Makes effective use of available technology, including, but not limited to, voice mail, e-mail, electronic medical record system, and other computer resources.

Emergency Services On-Call Clinician

Major Duties and Responsibilities:

  • Provide crisis intervention to individuals by telephone or face-to-face who are experiencing a mental health or substance abuse emergency.
  • Provide consultation, information, and support to individuals, families and to other community emergency service providers (e.g. Rockingham County Sheriff’s Department, Harrisonburg Police Department, Sentara Rockingham Memorial Hospital, Local Rescue Squads, ECC, Social Services Departments, etc.)
  • Perform pre-admission screenings for emergency hospitalizations, to include all admissions to private and state hospitals, both voluntary and involuntary.  In addition to screening the individual, this will encompass overseeing the petitioning process, advising the magistrate’s office regarding Emergency Custody Orders and Temporary Detention Orders, and coordinating/ensuring appropriate disposition/admission issues with respect to the most appropriate treatment in the least restrictive environment.
  • Perform assessments as requested regarding mental health and substance abuse issues. May include exploring and making dispositional arrangements for the individual in question.
  • Provide the Community Services Board with the required documentation of service delivery.
  • Participate in on-going coordination and consultation with other emergency service providers and with Community Services Board staff.
  • Participate in in-service training and supervision as required by the Supervisor of Emergency Services.
  • Perform other related functions as judged professionally necessary to ensure availability, response and continuity in the delivery of emergency services.

Regional Mobile Crisis Clinician

Major Duties and Responsibilities:

  • Provide face to face and phone contact with individuals, their primary support system, and relevant systems of care during a crisis.
  • Complete a crisis assessment, crisis safety plan, and report back to the RTCSB Regional Hub the disposition at the end of the crisis assessment and service.
  • Report back to the Regional Hub any mobile crisis responses provided to individuals diagnosed with developmental disabilities for referral to the Region I REACH program in order to assess for additional service needs post mobile crisis response.
  • Collaborate with Emergency Services when appropriate to provide coordination of care for the individual and their primary supports when appropriate.
  • Assess support needs and strengths of the individual.
  • Have a working knowledge of community resources and contact information for relevant systems of care, emergency, and supports.
  • Create individualized service plans to help stabilize an individual in crisis, regardless of disability.
  • Keep well-informed of proven practices for interventions for individuals in crisis across age and developmental span, with various needs
  • Provide follow-up care for up to 72 hours post crisis contact.
  • The crisis assessment and contact notes must be completed in EHR when an individual is provided crisis services. Same day contact notes must be completed for
  • Assures compliance with agency policy, confidentiality, client rights, licensure regulations and Medicaid regulations as applicable. Meets agency standards for staff productivity.
  • Maintain effective relationships with coworkers, consumers, and staff from other community agencies based on courtesy, compassion and respect.

  • Bachelors level in Human Services plus one year of experience in the human services field or a combination of education and experience that is equivalent is required.

Child and Adolescent Outpatient Clinician

General Statement of Duties:

To provide individual and family counseling, education, evaluation, assessment, and treatment planning for children, adolescents, and their families with mental health concerns. The Child and Adolescent Outpatient Clinician is directly responsible to the Supervisor of Children’s Clinical Services. He or she is expected to exercise considerable independence and autonomy in day-to-day decision-making related to the delivery of behavioral healthcare and to carry out those services on a daily basis with a minimum of supervision.
